- Parent or legal guardian must be present
- Please be sure to bring the required paperwork from the appropriate organization.
- If your child has preexisting health conditions (diabetes, heart conditions, seizures, or history of concussion, etc), please call the office before scheduling an appointment to talk to the provider.
- We offer on-site physicals for groups of 10 or more. Call for more information.
